INCENTIVE PROGRAM RULES
US CORE
1. Combined Insurance Sales Agents, Account Executives, Sales Coordinators, District Managers,
Market Directors, and Regional Directors are eligible to participate in Combined Insurance
Sales Incentive Programs. At its discretion, Combined Insurance reserves the right to change
the structure of the Sales organization, at any time for any reason. Combined Insurance sales
employees must be Full-Time, in Good Standing and not currently on a Leave of Absence.
2. Combined Insurance reserves the right to disqualify an individual if achievements and/or
performance fail to meet the Incentive Program Rules; the company’s Code of Conduct and/or
was not done in the spirit of the program.
3. Only Sales employees with a current Issue Rate of 85% or higher and a Second Month Persistency
Rate of 85% or higher can qualify for prizes and awards. The most current Underwriting Report
will be used at the end of this program to determine your current Issue Rating and 2 month

persistency.
4. Worksite AP will only count for 50% toward the qualification for this program.
5. Combined Insurance Sales employees who voluntarily or involuntarily leave the company during
or after a program forfeit their rights to any awards, prizes and/or trips they had not yet received
upon the last day of their employment.
6. Combined Insurance, at its discretion, retains the right to change, modify and/or delete
program(w) rules, employee eligibility, and/or terminate the program(s) at any time for any
reason.
7. Combined Insurance, at its discretion, retains the right at any time to substitute an award for
one of similar or equal value.
8. Sales to immediate family members during programs will not be counted toward prizes and
awards.
9. The date of the check for new policies paid by check must conform to the application date.
10. Medicare Supplement APV will only count for 50% towards the qualification for this program.
11. Program and contest rules have been modified to comply with the state of New York
requirements. As part of this change, the APV from the sale of life insurance policies will not be
counted towards incentive programs and contest awards for Sales Agents, Account Executives,
Sales Coordinators, District Managers, Market Directors, and Regional Directors in the state of
New York.
12. All sales are subject to verification Special Notes: APV to determine program winners: Winners’
APV will be determined based on submitted APV received and processed at the Combined
Insurance Processing Center. Official program numbers run two weeks after the close of the
program will be used to determine winners.
13. Awards shown are representations of actual awards. Prizes may vary.
