Benefit Plan Eligibility
All non-union full time employees and some non-union part time employees are eligible to participate
in all or some of the plans outlined in this Guide. A full time employee is an employee working a
minimum of 35 hours per week on a regular basis. An eligible part time employee is an employee who
works a minimum of 780 hours over a six month period.* Eligible dependents may also be covered
under the medical, dental and vision plans. These dependents include: your legal spouse, your
registered domestic partner and your children to age 26 (see specific plan for specific age
requirements). A waiver form is required if you choose to decline medical coverage.
When Coverage Begins
Coverage for Medical, Dental, Vision, Life/AD&D, Long Term Disability and Flex programs is effective
first of the month following date of hire, provided you have completed the necessary enrollment
forms prior to the deadline (30 days from date of eligibility). Short Term Disability coverage begins
after six months of continuous employment. 401k plan eligibility begins after one year of service and
1,000 hours worked in that year. Please see the 401k section of this guide for additional eligibility
requirements.
Employee Costs
All employees share in the cost (20-25%) of their plans via pre-tax payroll deductions.
Annual Elections
It is important that you choose your benefit elections carefully. Changes may generally only be made
during the annual open enrollment period (November for a January effective date of the following
year). The only exception is in the case of a change in family status (qualified life event).
* Part time employee hours worked are reviewed twice a year, in January and July. If an
employee has worked an average of 30 hours or more per week in the prior six months, they
are eligible for health benefits for the next six month period. However, if at the following six
month review, they have not worked at least 780 hours, they will no longer be eligible for
health benefits for the next 6 month period.
Example:
• Jane works 800 hours from January 1st to June 30th
• During the July hours review, it’s determined she is eligible for health benefits for the next six
months
• Jane works 400 hours between July 1st and December 31st
• During the January hours review, it’s determined she will not be eligible for health benefits
for the next six mon
