Qualified Life Event Changes
After you enroll in the benefits program, you typically cannot make changes until the next open
enrollment period. Exceptions include the Commuter Benefits Plan and the 401k plan. All other
benefits plans can only be changed if you experience an IRS-approved change in family status during
the year. Examples include:
• The addition of a dependent through birth, adoption or marriage
• The loss of a dependent through divorce or death, or if your child reaches the maximum age
limit for coverage.
• The loss of coverage as a dependent on your parents’ benefits coverage due to turning age 26.
• A change in your or your spouse’s employment status from full-time to part-time or vice versa
• A substantial change in your benefits coverage or a spouse’s coverage.
• A relocation that impacts network coverage
• You must notify the Human Resources Department within 31 days of qualified change.
• You may be required to certify your change in status.
