                              Among other metrics, VNSNY measures:

                              • Number of patients who choose Hospice at
                                Intake

                              • Number of patients who choose Hospice from
                                CHHA
                              • Number of patients who had signed Advanced
                                Directives as per the clinical record
                                documentation (case sampling/chart audits)

                              • 30 day hospitalization rate and ED visit rates
