Rehabilitation services :
• Rehabilitation services Provided by licensed physical, speech and
occupational therapists:
− Physical Therapy to continue to treatment for conditions impacting
mobility and strength
− Occupational Therapist to help the ability to perform activities of
daily living
− Speech Therapy for treatment for conditions which cause difficulty
with speech or swallowing.

Long-term care for people with debilitating
conditions:
• Personal care and companionship Hands on assistance with the
activities of daily living (ADLs) and additional support and monitoring for
high risk patients and those transitioning home from hospital discharge
− Help with mobility, bathing, dressing, toileting, food shopping, meal
preparation and light housekeeping to maintaining a clean, safe, and
healthy environment for the client
− Identifying, recording and reporting any changes in the patient condition or
behavior and ability to manage an emergency
− Recording and monitoring of vital signs
− Prescription reminders
Partners in Care − Help following planned exercises and special diets
− Respite care for caregivers when they need to attend to their own personal
commitments including taking vacation
