                           Telehealth at VNSNY


                           What is Telehealth?
                            Telehealth is a clinical intervention facilitated by technology

                            Used as a remote healthcare tool to monitor, assess and educate patients in their
                             home


                            Patient readings may include blood pressure, weight, pulse, pulse oximetry and
                             blood glucose. In addition, patient symptom data are transmitted via interactive voice
                             response (IVR) technology


                            VNSNY nurses continue to visit patients in their homes with the added benefit of
                             enhanced monitoring via Telehealth between home visits

                            Used primarily for patients with Heart Failure and Myocardial Infarction and
                             comorbid conditions such as COPD, DM and HTN

                            Reduction in  ED visits and Hospital readmissions


                            Increase in patient adherence to care plan

                            Provides opportunity for early detection and intervention
