Telehealth at VNSNY
What is Telehealth?
Telehealth is a clinical intervention facilitated by technology
Used as a remote healthcare tool to monitor, assess and educate patients in their
home
Patient readings may include blood pressure, weight, pulse, pulse oximetry and
blood glucose. In addition, patient symptom data are transmitted via interactive voice
response (IVR) technology
VNSNY nurses continue to visit patients in their homes with the added benefit of
enhanced monitoring via Telehealth between home visits
Used primarily for patients with Heart Failure and Myocardial Infarction and
comorbid conditions such as COPD, DM and HTN
