                            - Reaches out to the Field Clinician and facilitates or takes appropriate actions as needed

                            Customer Care Center (CCC)Nurse  conducts monitoring of Telehealth Alerts via the
                             AMC Portal
                            CCC nurse calls the patient and triages for appropriate action
                            CCC nurse enters Telehealth Alert Coordination note for follow up and actions taken
