Standard (Default) Parameters

Weight gain/loss of ≥ 3 lbs in 1 day or ≥ 5 lbs in 7 days
Oxygen saturation ≤ 89
Blood glucose > 300 mg/dL or < 60 mg/dL
Blood Pressure
- Systolic > 200 mmHg or < 91 mmHg
- Diastolic > 110 mmHg or < 40 mmHg
Heart rate > 110 BPM or < 50 BP

Note: The physician may also provide patient-specific parameters

What to order from Contract Administration

1.Telehealth Monitor- order for all patients
2. Scale - order for all patients (450 lbs maximum weight)
3. BP monitor- order for all patients
4. BP cuff (S, M, L, XL)- order appropriate size based on
patient’s shirt size. (XL largest size available)
5. Pulse oximeter- order if needed with or without home O2
6. Glucometer- order separately if needed for patients with Diabetes
Glucometers are not provided by the Telehealth vendor
7. Glucometer blue tooth adapter- always order if the patient requires
telehealth blood glucose monitoring

Note: Do not Order Telehealth equipment in Parachute
