Inclusion Criteria

 Heart Failure in any position or Myocardial Infarction

 Agreeable to Telehealth and to responsibility for self-monitoring

 Functionally & cognitively able to participate or has a care partner who can
assist

 Has direct access to land line or cell phone

 Able to stand on scale for at least 40 seconds

Note: May exclude use of scale if patient is unable to stand
and will benefit clinically from use of other TH devices)
Note: Must also meet Payor requirement (Refer to Slide # 17)

Exclusion Criteria

 Patient/caregiver declines participation

 Weighs more than 450 lbs (maximum for scale)

 Requires larger than an extra large BP cuff

 Not functionally or cognitively able to participate or has
no caregiver assistance
