VNSNY’s Intake Department
sits at the core of the
Intake process, and utilizes
advanced technology
to provide an e cient,
responsive, and personalized
process. Patient services are
tailored in order to provide
suitable VNSNY programs,
assuring each patient receives the safest level of home care.
VNSNY’s Intake Department is open 7 days a week, including
weekends and holidays. Weekday hours from 8:30 am to
5:30 pm and weekends/holidays are from 8:30 am to
4:30 pm. For more information or to make a referral, call
212-609-7300. A referral may also be faxed to 212-290-3939
or emailed to mdhotline@vnsny.org.
