Center for Home Care Policy & Research

The Center for Home Care Policy & Research is recognized
as the pre-eminent research center for home- and
community-based care and is the only one of its kind in
the nation. The aim of the Center is to support the highest
quality, most cost-e ective care at VNSNY and to build
knowledge for positive changes in health care practice
and policy at the community and national level. Studies
and analyses are designed to inform and improve practical
decision making by clinicians, patients and families,
communities, and policy makers, with the focus on e ective
and a ordable ways to improve people’s quality of care and
quality of life at home in their communities. Focus is on:
• Improving care for diverse populations with
complex care needs
• Helping clinicians and patients translate scienti c
evidence into everyday practices to help them
manage chronic conditions
• Supporting communities that promote successful

aging in place
• Analyzing and informing public policies that
a ect home-based and long-term care
• Assisting in developing and evaluating interventions

and models to improve care
