Partners in Care provides registered nurses, who are
experienced in disease-speci c care and the care of medically
complex patients, with private duty nursing services not
covered by most insurance. This can help reduce hospital
length of stay, assist with safe transition across settings and
minimize setbacks or potential risk of infection. Flexible,
customized care can be arranged for short-term or
ongoing needs, including:
• Coordination of care and assistance with following

doctors’ orders
• Pain management and medication reconciliation
• Supervision and clinical treatment for continuous care

needs, such as:
- Infusion therapy
