Partners in Care provides
customized home health
aide services for patients
who want assistance
with the activities of daily
living beyond Medicare
or other insurance
coverage. We carefully match patients with a highly trained,
certi ed home health aide—or a team of aides—to provide
assistance with activities of daily living. A registered nurse
supervises the aide, oversees the care plan and updates it
as necessary. Flexible scheduling is available, from hourly
service through round-the-clock care, for short- or long-term,
periodic or continuous care needs. Services include:
• Reminders to take medications
• Food shopping, cooking and meal preparation
• Ambulatory escort at discharge or to and from medical

and other appointments
• Recording vital signs
• Bathing, dressing and grooming
