VNSNY CHOICE Total

VNSNY CHOICE Total is a Medicaid Advantage Plus plan
that brings together all of the bene ts of Medicaid Managed
Long Term Care and Medicare Advantage into one plan.
Members have one phone number to call, one ID card
and one care management team.
The plan is designed for individuals who require ongoing
help with the activities of daily living in order to remain
safely at home and stay out of a nursing home. It provides
all the bene ts covered under traditional Medicare, plus:
• Extra bene ts that were designed to enhance access to
health care services
• A comprehensive prescription drug bene t with a broad
formulary of covered drugs
• Care management by a clinical team, to help members
maximize their ability to manage chronic illnesses and
avoid hospitalization
• Long-term services and supports that members need to
remain safely in their homes. These may include home
health care, community-services, transportation, social
and environmental services and more
