VNSNY CHOICE Medicaid Managed
Long-Term Care (MLTC)

VNSNY CHOICE MLTC is a program for individuals who
wish to remain in their homes and communities but require
ongoing assistance due to chronic illnesses and functional
decline. The program includes home, community, and
facility-based long-term care and health-related services.
VNSNY CHOICE MLTC emphasizes prevention and fosters
member independence through a highly skilled clinical
team and member engagement with their Nurse or Social
Worker Care Coordinator. Care Coordinators develop and
coordinate a personalized plan of care to meet member’s
health needs, while working in collaboration with the
member, his/her family and the physician. Members
can continue to see their own doctors, including their
primary care physician and any specialists that members
might require.
