VNSNY CHOICE SelectHealth
is a Medicaid Special Needs
Plan designed to meet the
specialized needs of people
who are living with HIV,
identify as transgender or
are homeless. Dependent
children of members are also
eligible to enroll.
The plan provides all of the bene ts of Medicaid, including
medical, pharmacy, behavioral health/substance use, vision,
dental and specialized services such as access to Health
Homes and home- and community-based services.
Members who are living with HIV have an HIV specialist as
their Primary Care Physician. Every member has a team of
caregivers, led by a Care Manager. The plan puts emphasis
on removing barriers to care, and the network includes
expert providers who embrace diversity in sexual
orientation, gender identity and gender expression.
