Hospice
Project HOPE
In 2015, VNSNY Hospice and Palliative Care launched Project
HOPE (Hospice Outreach, Provider and Patient Education
and Engagement) to increase awareness and utilization of
hospice services in minority and low-income communities in
Harlem. In these communities, hospice care is under-utilized
largely due to access to care barriers, misconceptions and
myths about hospice.
The Project HOPE model provides customized support
to hospitals and other healthcare facilities with education
programs that debunk myths and improve service
coordination, including:
• How to have the conversation about hospice eligibility
and services available with patients and their families
• Outreach to physicians, nurse practitioners, residents,
social workers and other clinical providers
• Providing patients with advanced directive and
end-of-life education
• Developing educational programs, including
conducting grand rounds on end-of-life care
