VNSNY Hospice and Palliative Care services are designed to
improve the quality of life for patients and families living
with advanced and life-limiting illnesses. Care is provided in
the patient’s home, whether it is a private residence, nursing
facility or our hospice residence. In certain situations, care
is provided in a hospital setting. Our interdisciplinary team
of experts work to:
• Manage discomfort promptly
• Reduce unnecessary hospitalizations
• Provide emotional and spiritual support

• Streamline coordination of services with
healthcare providers
In situations that require urgent attention, VNSNY may
implement a Community Paramedicine Program where
a specially-trained paramedic will work with hospice
physicians to alleviate symptoms and help prevent
hospitalizations.
VNSNY has specialized hospice care programs that provide
an enhanced level of care, including programs for cardiac
conditions, Chronic Obstructive Pulmonary Disease (COPD)
and wounds.
