Care Management

The Care Management program from Partners in Care
enhances recovery, safety and satisfaction for patients
with complex care needs. This is bene cial for frail, elderly
individuals with chronic conditions and particularly helpful
for caregivers challenged with providing care, so patients
can remain safe at home for as long as possible. A registered
nurse case manager will assess, coordinate, and manage
a range of services including:
• Comprehensive evaluation of health, psychosocial,
functional and environmental factors for patients
needing help with IADLs
• Expert advocacy, education and oversight of
interdisciplinary team
• Care coordination for assistance with ADLs and IADLs,
medication management, nutrition, home safety and
household arrangements
• Family caregiver education and status updates
• Assistance navigating elder care options, associated
paperwork and connecting to community resources
