                                      Why Advance Care Planning?




                                   • Introduction of Advance Care Planning is important
                                     for all patients upon admission to the CHHA


                                   • Identification at point of Intake of patients who need
                                     an in-depth advance care planning conversation, or
                                     who have palliative or hospice needs


                                   • Enhanced clinical assessments by CHHA clinicians of
                                     advance disease if appropriate, to better identify
                                     patients with palliative or hospice needs














                                      Palliative and Hospice Care-
                                      What’s the difference?


                                                               Palliative care is focused on providing
                                                               patients with relief from the symptoms,
                                                               pain, and stress of a serious illness -
                                                               whatever the diagnosis

                                                               Palliative care is appropriate at any age and
                                                               at any stage in a serious illness, and can be
                                                               provided together with curative treatment
                                                               Hospice Care provides compassionate,
                                                               comfort-oriented end of life care aimed at
                                                               improving quality of life

                                                               Hospice care settings can include the
                                                               patient’s own home, a nursing
                                                               home/residence, or a specialized in-patient
                                                               unit
