Learning Objectives

At the end of this course the learner will be able to:

 Describe the VNSNY Advance Care Planning
 Identify the core and disease-specific indicators of advanced disease
 Use the Palliative Performance Scale and other core and disease-specific
indicators to identify patients with advanced disease who would benefit from
enhanced advance care planning or palliative care interventions
 Apply best practices for communicating a patient’s palliative or hospice
needs to the referring healthcare team and/or the patient’s PCP
 Follow process for documenting assessment and communication in
coordination notes
