• Assessment at Intake will now include a screening
for potential palliative or hospice needs
• Communication with referrer and the patient’s PCP
will now include your findings on palliative or
hospice needs
• Documentation of PPS score and other findings in
coordination notes
• Communication to CHHA clinical team on any
advance care planning, palliative or hospice needs
