Assessing the Need for Other Disciplines

Patients admitted to Home Care may be assessed and referred to Other Disciplines by the
clinician:
• At Start of Care (SOC)
• At any point during the episode of care

Other Disciplines include:
• Physical (PT)
• Occupational Therapy (OT)
• Speech Language Pathology (SLP)
• Medical Social Work (SW)
• Home Health Aide (HHA)
