Referring to Other Disciplines

• During the Admission Visit, the clinician will:
a. Choose Pathway “Need for other
disciplines”, indicate which discipline
b. Save the order
c. Plot ONLY the Evaluation visit on the
calendar and attach “01” Service code for
the needed discipline (PT01 for Physical
Therapy; OT01 for Occupational Therapy; or
ST01 for Speech Therapy, MS01 for Medical
Social Work).
• During a Subsequent Visit, the clinician will:
a. Complete a New Order and choose
“Physician Order” for the specific .
b. Add reason for referral in “Instructions”
c. Add the ONLY the Evaluation visit to the
Calendar and attach the appropriate
service code
