Areas of focus for Medical Social Work

• Concrete needs (food, clothing, shelter, medication, rental or utility arrears etc.)
• Assessment/linkage for public benefits, Community referral
• Home environment concerns (adequate for care, safety, accessibility, sanitation)
• Coping/adjustment to illness (incl. disease specific supports)
• Limited support system/social isolation
• Cognitive/Psychosocial Status (anxiety, depression, alcohol/substance abuse, etc.)
• Long term care options
- Assisted living, MLTC, Nursing home etc.)
• Advance directives/care planning
(Health Care Proxy, Living Will, DNR, Power Of Attorney)
• End- of -Life counseling and options (Hospice)

Home Health Aide (HHA)

• If the clinician needs to order an HHA for the patient, MED/SURG Nursing
Pathway MUST first be selected
• Select “Need for other Disciplines”
• Select Home Health Aide
• Complete the Aide Care Plan
• Enter the Aide frequency to the calendar
• Call Contract Administration to place the HHA Order approximately 30
minutes after Synching device
