Criteria for Medical Social Worker

• Doctors Orders:
• Social Worker visits 1-2 times/month or for the cert period
• Reason(s) for requesting SW services

• Social Work: is not a freestanding service in a Certified
Home Health Agency.
o There must be a skill for the RN and/or PT in order for the Social Worker to be
placed
o Services can only remain in as long as nursing and/ or PT are still Providing
skilled services

• Requires authorization for Managed Care insurance
