PERISTOMAL ASSESSMENT
Peristomal skin should be intact. To prevent or manage
complications :
• Measure stoma to resize the appliance
• Assess patient sitting/lying down and standing to
assess for abdominal folds and creases that can be
complicating pouch adherence and resulting
peristomal problems
• Product selection may entail use of convex or flat
appliances. Accessory products to improve the fit of
the appliance may also be required
• Refer to WOCN for assistance either by calling or
request in HCHB coordination notes (Wound Consult
Request).
