                            Note the supplies that the patient received in the hospital
                              and place an order in parachute (check referral comments)
                           ** The clinician can only order the supplies after the SOC
                           visit  is completed.
                            Type in the product and name /description of the wafer,
                              pouch or ostomy accessory.
                            Or enter the product number written on the box or on the
                              ostomy pouch.

                           **Do not change the type of pouching system that the
                           patient is discharged with unless it is no longer
                           appropriate
