                           Quality Measures


                             Quality measures are based on information collected from the CMS-
                                mandated Outcome and Assessment Information Set (OASIS)


                             The 12 quality measures include:
                                 • 3 measures related to mobility
                                 • 4 measures related to meeting basic ADLs

                                 • 3 measures related to medical emergencies, such as:
                                     o Readmission to the hospital
                                     o Requiring unplanned medical care
                                     o Unplanned medical care specifically related to a wound
                                 • 2 measures related to how home care ends such as:
                                     o Patients who stay at home after discharge from homecare
                                     o Patients whose wounds improved after surgery
