Behavioral Health

Care, counseling and coordination are provided by a team psychiatric
and specialty-trained RN’s. The program is administered in the home
and includes:

 Comprehensive assessment of medical, functional and psychological debility

 Patient education in self-management techniques

 Skilled counseling

 Management of psychotropic medications

 Evaluation and management of the plan of care

 Linking patients and families to local community resources for ongoing
support
