Medicare Notices Reminders
The patient has a paper copy of all the forms in the SOC folder.
Forms will be submitted electronically to the office when you Sync.
Any paper forms should be submitted to the office manually.
Issue the Medicare notices in a timely manner
Explain the appeal process when serving the Generic Notice
• If the patient or family is planning to appeal, notify your manager
as soon as possible.
• If the patient is planning on appealing, have all notes completed
for this patient. Livanta will ask for the complete chart as soon as
they receive the call from the patient who disagrees with the
discharge from VNSNY. The documentation should support the
decision for discharge.

- “Buddy Codes”
Clinicians can use buddy codes to remind them to issue Medicare notices.
They can “schedule” these buddy codes to their visits:
HHCCN buddy code for a RN19 (DC of services, not from agency) visit
NOMNC buddy code for a RN11 subsequent visit which is at least 2
calendar days before the discharge date.
For example if the last scheduled RN visit is July 28 and the PT’s last visit

is on the 29 , the RN needs to issue an HHCCN on his/her last day.
If the RN was the last discipline on the case, and his/her last day was on
July 28, then he/should issue the NOMNC at least 2 calendar days prior.

Should he/she issue it on the 27 ? NO. He/She would have to issue it on
the 21 .
