                           What are Conditions of Participation (CoP)?



                              The Conditions of Participation (CoPs) are Regulations that
                              Home Health Agencies (HHAs) must meet in order to
                              participate in the Medicare and Medicaid programs.


                              The requirements focus on the care delivered to patients by
                              Certified Home Health Agencies and reflect:
                                  • patient-centered care
                                  • data-driven
                                  • outcome-oriented process
                                  • Promotion of  high quality patient care at all times for
                                    all patients
                               DOH helps to ensure that CoPs are being followed

                              Admission To Home Care Policy


                            Purpose:
                             To delineate standards and a process by which a patient
                               can be evaluated and accepted for admission.
                             To comply with Medicare Conditions of Participation (CoP)
                               for Home Health Agencies:
                                • Acceptance of patients, plan of care and medical
                                  supervision
                                • Patients are accepted for treatment on the basis of a
                                  reasonable expectation that the patient’s medical,
                                  nursing and social needs can be met adequately by the
                                  Agency in the patient’s place of residence.
