VNSNY Discharge Policy and
Procedure from Home Health Care
The Visiting Nurse Service of New York has 1 policies and 1 procedure in place
for discharging patients from Home Health Care.
Policy:

community or other health care provider

Purpose:
• To ensure and appropriate, timely and safe transition for the patient to the
community or other health care provider.
• To provide essential patient information to ensure continuity of patient care.

VNSNY Admission Criteria
