Integumentary Command Center
The ICC is the section where the clinician can enter wound information, if applicable. The rehab
clinician will only observe and document wounds, not manage or treat them. If there is a need for
wound care, a nurse should be ordered.
If there are wounds present, click on Add New and describe the location of the wound. As
mentioned earlier, the pathway for Altered Skin Integrity should have been chosen to document this
wound and to observe and assess the wound in subsequent visits.
