Click on the next required box, Assessment. Since the rehab orders are only for observation and
assessment, rehab clinicians are not required to perform other measurements, e.g. measurements,
tunnelling, etc. Click on select “Not Assessed” in this section, and choose the appropriate reason
for not assessing, e.g. Covered by non removable dressing, or Not ordered this visit.
Click on the last required box, Wound Care Provided. Rehab clinicians will not provide wound
care. If wound care is needed, nursing services should be ordered. Select “Care Not Provided”
and select the appropriate reason.
