Primary Care – Summary
There are 2 Family Health Teams across 8 sites at SJHC and SMH that together have
over 130,000 patient visits per year. Family health teams are primary health care
organizations, funded by the Ontario Ministry of Health and Long-Term Care, that include a
team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and
other professionals who work together to provide primary health care for their community.
