10-30% market share in CCC cases across
its top LHINs.

• The Scarborough Hospital is the top referrer for Orthopaedic Rehab & CCC, followed by

Michael Garron Hospital & North York General Hospital

• Most rehab patients are discharged with home care or to ambulatory care services. Most
CCC patients are discharged to other institutions including Michael’s Garron & North York
General Hospital.

• Most orthopaedic rehab and CCC cases are for Fracture of Lower Extremity and
Replacement of Lower Extremity
