6.  OUR SHARED ACCOUNTABILTY: PERFORMANCE REPORTING AND

                   EVALUATION

               The three organizations have strong relationships with the Toronto Central LHIN, Ministry of Health and
               Long-Term Care, Health Quality Ontario and Canadian Institute of Health Information. All three also
               demonstrate a commitment to quality and safety through regular accreditation by Accreditation
               Canada. Through these partnerships and on respective websites, the three organizations publically
               report their performance. In keeping with their commitment of being open and transparent, the new
               health network will also build a performance measurement framework that will follow a balanced
               scorecard approach and examine metrics in areas such as access, quality and patient safety, research
               and education, financial sustainability and our people.


               Operational savings as a measure of success
               All three hospitals have come together recognizing not only the positive patient care impact to
               integration but the needed savings to ensure sustainability in the face of declining hospital revenues and
               growing fixed expenses.

               The financial analysis measured operational savings that could be achieved through integration. This
               range of savings will be achieved over the course of three years as the new network transitions to a
               steady state. Those savings ($8.8-$14.3 million) will be tracked as a performance measure of the success
               for the new network. Moreover, the savings will be reinvested into patient care services to maintain
               support for existing programs and priorities – with the expectation that as patient volumes and
               complexity is also rising, these savings will serve to mitigate those challenges in order to maintain
               current patient care volumes.


               Referral patterns as a measure of success
               The proposal for voluntary integration is based upon the assumption that all three organizations’ sites
               would remain in place and will continue to service each local community. The three organizations intend
               to maintain the current level of services and referrals provided to our communities. In particular,
               Providence Healthcare will maintain commitments outlined in the Memorandum of Understanding with
               each of its five key referral partners. The new health network will continue to monitor referrals to
               ensure ongoing, consistent access to services.

               TC LHIN reporting
               We are also aware of the importance of measuring the impact of the integration itself.  In working with
               our partners at the TC LHIN, we have identified the following metrics and targets that will be measured
               and reported on both publically and to the TC LHIN over the next three years.
