Drug Related Problems at home in neurological patients

Background: Drug related problems (DRPs) at home in neurological patients
was studied in 70 patients after discharge from Chiangmai Neurological
hospital, Chiangmai.
Objectives: The objectives of this study was to determine the types of
problems in order to improve drug system.
Methods: The data were collected between January – September 2014.
Results: The results showed 70 DRPs from 60 patients (63.5%), 1.2 problems
per patient. The most of DRPs was non adherence (48.6%), adverse drug
reactions (11.5%), others were inappropriate drug storage, elderly patients
cannot take care themselves.
Conclusion: Pharmaceutical care at home could solve the problems because
the behavior and condition at home might be different from hospital.
