Voluntary Life Insurance Coverage
TNJH offers you the option to purchase voluntary life insurance for yourself, your spouse, and dependent child(ren) through Cigna. You
must elect coverage for yourself in order to elect coverage for your spouse and/or children. You pay 100% of the cost for the coverage
which will be deducted from your paycheck on a post-tax basis. If you wish to apply for voluntary term life, or increase your current
coverage, an Evidence of Insurability (EOI) form will be required. Coverage applied for is not effective until the EOI is received and the
coverage amount is approved by Cigna.
For Yourself – You can purchase increments of $10,000 up to the lesser of 5x your annual salary or $500,000.
For Your Spouse – You can purchase increments of $5,000 to the lesser of 100% of your employee life election or $125,000.
For Your Children – You can purchase $4,000 for your child(ren) up to age 23 or 25 if a full-time student. The benefit amount for
children at least 14 days and less than 6 months of age is $1,000.
Voluntary Long Term Disability Insurance Coverage
The voluntary long-term disability plan is designed with benefits beginning after a 180-day “elimination period.” Your monthly benefit
options are 50%, 40% or 25% of your monthly earnings to a maximum benefit of $5,000. If you wish to apply for Voluntary
LTD coverage, an Evidence of Insurability (EOI) form will be required. Coverage applied for is not effective until the EOI is received
and the coverage amount is approved by Cigna. Pre-existing conditions that lead to a disability that begins during the first 12 months
of coverage will not be covered. Your payroll deductions are made post tax, so any benefit you receive will not be considered taxable
income.
