Lost Patient Log

               The purpose of this log is to identify patients who have not had an appointment in a number of
               weeks, but whose cases have not been discharged. WebPT Reach will send an automatic “Lost
               Patient” email when a patient has not been seen in 2 weeks, but the clinic can also call or email
               (using the “Touching Base” canned email) to check in with a “lost” patient.

               To generate the Lost Patient Log:
                   1.  In WebPT under the blue Reports sidebar, click Lost Patient Log.













                   2.  In the Lost Patient Log window:
                          a.  Leave the boxes for “Beginning Date” blank.
                          b.  In the box below “Days Since Last Note” input 90.
                          c.  Change the Therapist to print an individual report for each PT.
                          d.  Generate Report and a list of patients will populate.








                   3.  After the list of patients has loaded, click Generate PDF.
                   4.  Sort the list by No Appointment.
                   5.  Print and distribute a list to each PT, who will review and return to the Clinic
                       Coordinators after notating patients who need to be contacted and those whose cases
                       they will discharge.
                   6.  Create a Chart Note after touching base with the patients to detail any communication,
                       and inform the PT of their status.
