Utilizing the Authorization Log

               Each clinic has an Authorization Log in Google Drive that is used to track the status of
               patients’ authorization submissions and responses. This log should be updated on a daily basis
               by the daytime Clinic Coordinator to include any patient who will need to have an authorization
               request submitted after reviewing the schedule and the Ending/Expired Authorizations reports
               from the WebPT Dashboard.

               The Authorization Log will include the following information:
                          ●  Patient’s First Initial & Last Name
                          ●  Appointment Type
                          ●  PT Name
                          ●  Date of Eval/Re-Eval
                          ●  Authorization Type
                          ●  Date the authorization request was submitted
                          ●  Authorization Reference Number (PCN number for Optum)
                          ●  If Approved (Yes or No)
                          ●  Approval or Response Number
                          ●  Notes









               The log will be sorted by monthly sheets in order to easily view and track the status of requests.
               The tab of an open sheet will display in white along the bottom toolbar. Click other tabs to
               change sheets. To create a new month, click the + icon to Add Sheet and Rename
               appropriately.








               ●  A patient’s information should be recorded on the log as soon as auth is required.
               ●  Upon submission of a request, update the Date Submitted and Reference (Auth) # fields.
