How to Verify UMR

UMR is a United Healthcare third-party administrator. To verify UMR plans, you will call UMR
and request a faxed copy of benefits, which will contain a patient-specific passcode number that
you will then use to call and speak with a representative. You can view an outline of the benefits
through the online portal.

To request a fax of benefits:
1. Call UMR Provider Services at 877-233-1800
2. Press 1 for Medical questions
3. Enter the patient’s member ID #
4. Enter the patient’s date of birth
5. Press 1 to receive a benefits & eligibility fax
6. Enter 10-digit fax #
7. Once the fax is received, review for benefit information. Call the phone number provided
on the fax and follow the prompts to enter the passcode number and connect with a
representative.

Reminder! Any correspondence with insurance representatives must be detailed as a Chart
Note in the patient’s WebPT chart and should include the representative’s name and a call
reference number.

To access the online portal, go to UMR Portal and click the blue Login/Register icon in the top
right corner. Log in with username and password from Master Login List.
