In the top Policies/Key Dates box, confirm the policy has Active Coverage. The Effective -
Term Dates displays when the plan became active and if/when it will terminate.
In the Patient/Provider Information box, the Plan Requirements will indicate if a Referral is
required. Prior Authorization & Notification requirement must always be checked through
Optum.
Review the information in the Selected Insurance Details box.
