Under Plan Deductibles & Maximums, review to see if the Individual In-Network or Family In-
Network Deductible has been met (benefits will state if deductible applies), and if either Out of
Pocket Maximum has been met.
In the Detailed Benefits Information box, click on PT/OT/ST or Rehabilitation Services
Outpatient Therapy.
