How to Verify Tricare

               Tricare benefits will be verified by calling Humana/Tricare and via the online portal.

               To verify via automated information and to connect to a representative:
                   1.  Call 800-444-5445
                   2.  Press 2 for calling from a Medical Facility
                   3.  Press 2 for All Others
                   4.  Press 2 to Verify Your Patient’s Eligibility
                   5.  Enter the Tax ID #
                   6.  Enter the patient’s 9-digit sponsor ID (SSN) or 11-digit D.o.D. benefits #
                   7.  Enter the patient’s date of birth
                   8.  Press 2 to hear eligibility information
                   9.  To speak to a representative, return to the main menu & press 7 for All Other Questions

               To view an outline of benefits using the online portal, go to Humana Provider Portal.
                   1.  Log in with the User ID and Password found in External Documents under Master Login
                       List.
                   2.  Search for the patient by entering their Tricare ID and Date of Birth, or by Auth/Order
                       # if the clinic received one. Click View.

















               The Patient Profile will provide information outlined in the following sections:
                   ●  TRICARE Eligibility Information
                   ●  Sponsor Information
                   ●  PCM (Primary Care Manager) Information
                   ●  Referrals & Authorizations
                   ●  Catastrophic Cap (Out of Pocket Maximums) & Deductibles
