One year prior to your loved one’s graduation, you should be coordinating with and visiting DDA Providers
to research the services you are interested in. These visits are for your loved one and the provider to discuss
and ensure access to the appropriate programs. During these visits, be sure to stay in communication with
your Coordinator of Community Services as there will be application and intake deadlines for the providers
you select. Your Coordinator of Community Services will also reach out to your choices and eventually come
to an agreement with one of them.
Overall, this is a snapshot of what Transitioning Youth Services/Planning looks like and I have provided sev-
eral links as resources for you. If your loved one is eligible for DDA services and would be interested in re-

Karen Lopez, COO, has over 20 years of knowledge and experience in special education and health and human ser-
vices. She has spent her career dedicated to helping children and adults with special needs be successful, both in
school settings and in their communities. Karen started working at EPIC in April 2019 and works diligently to help
lead, educate, motivate, and encourage her team so that EPIC continues to provide excellent customer care to the
people we support.
