Medical Insurance

United Healthcare | PPO Medical Plan
The United Healthcare Preferred Provider Organization (PPO) plan allows you to direct your own care. You are not limited to the
physicians within the network and you may self-refer to specialists. If you receive care from a physician who is a member of the
PPO network, a greater percentage of the entire cost will be paid by the insurance plan. You may also obtain services using a non‐
network provider; however, you will be responsible for the difference between the covered amount and the actual charges and
you may be responsible for filing claims.

United Healthcare | HSA Medical Plan
With the HSA plan through United Healthcare, you can pay for qualified healthcare expenses now and grow your savings for future
healthcare needs. This plan combines a High Deductible Health Plan (HDHP) with a special, tax-qualified savings account (HSA). You
can contribute tax-free money to your HSA up to IRS maximums. The money in your account is yours to pay for current healthcare
expenses - or you can save toward future healthcare expenses. Similar to the PPO plan, you have the freedom to choose your
doctor without the requirement of selecting a PCP and you may self-refer to specialists. You may use a network provider whose
negotiated rates provide richer levels of benefits with claim forms filed by the providers. You may also obtain services using a non-
network provider; however, you will be responsible for the difference between the covered amount and the actual charges and
you may be responsible for filing claims.

Go to http://choiceplus.welcometouhc.com/ , scroll down, look to the right and select, “Find Your Doctor.”

United Healthcare | SimplyEngaged & Rally Wellness Programs

Take care of your health and be rewarded! SimplyEngaged is a personal health and wellness program which allows you to earn
rewards when you complete the health actions below. Eligible participants include you and your spouse.

It’s Easy to Get Started:

Participate in biometric health screening - learn about your numbers
Access the biometric screening options through Rally when you log in to www.myuhc.com $75 reward

Complete an online health survey through Rally when you log in to www.myuhc.com
within 90 days of the start of the program
Complete the survey and receive your results as a “Rally Age” - an indicator of how your $25 reward
health compares with your actual age

Visit a participating fitness center at least 12 times per month
Register at a participating fitness center or YMCA through the Health & Wellness tab on $20 per month
www.myuhc.com; present your fitness ID card each time you go to the gym reward

Complete a telephone-based health coaching program
Complete the health survey first, then call the health coach at (800) 478-1057 to begin $75 reward
working on your plan - plan accordingly (this step takes 3 to 5 months to complete)

Complete at least three Missions through the Rally experience
Complete the health survey to receive suggested online “missions” (this step takes at $50 reward
least 4 weeks to complete)

Maximum Reward per Team Member = $200 | Maximum Reward per Family = $400
