Voluntary Benefits
Voluntary benefits help protect you and your family from unexpected out-of-pocket expenses. You are able to purchase these
products from Transamerica at discounted group rates and special guarantee issue during your initial eligibility window. The
Transamerica offering includes Hospital Indemnity, Critical Illness, Accident, and Universal Life with Long Term Care Insurance. Your
premiums are paid through payroll deductions on a after-tax basis. All of the Transamerica policies are portable, which means that
you can keep them should you change jobs or retire, with no increase in premiums.
Transamerica | Hospital Indemnity
Hospital Indemnity insurance coverage provides you with payments when you are admitted and when you are confined to a
hospital, due to a covered accident or illness. Typically, a flat amount is paid for admission, and a daily amount is paid for each day
of a hospital stay. Hospital Indemnity also pays extra benefits for admission to or confinement in an Intensive Care Unit (ICU), and
for other benefits and services. Payments are made directly to you to use as you see fit. With Transamerica, you’ll have a choice of
two comprehensive plans which provide payments in addition to any other insurance payments you may receive, and no
coordination of benefits!

Transamerica | Universal Life with Long Term Care Insurance
Universal Life insurance helps protect your loved ones for your entire life, not just during your employment, if something should
happen to you. Purchasing Universal Life Insurance, may be a great supplement to our company-paid Basic Life and AD&D plan
because it provides coverage for your entire life, not just while you’re working. Universal Life Insurance is a policy that you own; it
can never be canceled, even if your health changes. Because you own it, you can take it with you when you change jobs or retire
with no change in the premium. It helps ensure that short- and long-term financial obligations could be taken care of if something
unforeseen should happen.
Built in Long Term Care (LTC) Benefits
Accelerates a portion of the face amount of 4% for confinement (assisted or skilled nursking) and 2% for adult day care / home
healthcare. Requires the loss of 2 activities of daily living (bathing, dressing, toileting, transferring, maintaining continence).
The Transamerica policy includes an extension of benefits rider—meaning, if the LTD benefits exhaust the face amount of the
policy, the policy will generate 100% of the face amount to continue to pay LTC benefits.
Cash value and minimum interest rate accumulation
The policy build cash value that you may borrow from as long as there is cash value in the policy. If you miss a premium
payments, the cash value can also be used to pay your premiums to keep the policy active. Any loans that are not repaid, will
subtract from the total face value of the policy. The guaranteed minimum interest rate is 3%
