In order to receive the full value of your plan, schedule your preventive care exams! Our medical plans cover these exams
100% when you use in-network providers. Preventive exams can help identify any potential health problems early on. Not
all preventive care is recommended for everyone, so talk with your doctor to decide which services are right for you and

WITH A BOARD CERTIFIED DOCTOR
If you need care for things like flu, a cold, sinus infection, pink eye, rashes, fever and more, a LiveHealth Online doctor will be able
to assess your condition and provide treatment options and even send a prescription to the pharmacy of your choice if needed.
They are there to assist you 24/7.

HOW IT WORKS
First you need to sign up, which is quick, easy and free to do. Each time you log in, you'll see doctors who are available in your
state. Simply click on a doctor’s photo to view his or her star rating, profile and languages spoken. Want to come back later?
Bookmarking a doctor's profile is easy. Once you select a doctor, click Connect and in just a few minutes you’ll be talking with him
or her – face-to-face! During your session, the doctor can review your health history, answer questions, assess your condition and
even prescribe prescriptions if needed.

WITH A LICENSED PSYCHOLOGIST OR THERAPIST
If your are feeling stressed, worried, or having a tough time, you can talk to a licensed psychologist or therapist through video using
LiveHealth Online Psychology. In most cases, you can see a therapist within four days or less. Log in and choose the therapist you’d
like to see then you’ll get an email confirming your appointment.

HOW IT WORKS
Make your first appointment but using the mobile app, online website or call LiveHealth Online at
1 (844) 784-8409 from 7 am to 11 pm. You will then get an email confirming your appointment.
You must be at least 18 years old to see a therapist online and have your own LiveHealth Online
account. Psychologists and therapists using LiveHealth Online do not prescribe medications. A
session usually lasts about 45 minutes.

WHAT WILL A VISIT COST?
Both LiveHealth Online options will cost $10 copay if you are enrolled on the medical plan.
